Ruptured Aneurysm of the Sinus of Valsalva with Ventricular Sepeal Defect

Abstract
A case of a ruptured aneurysm of the sinus of Valsalva with a ventricular septal defect in a 20 year old college student is presented. A right heart catheterization revealed an equivocal evidence of a left to right shunt at ventricular level and a definite evidence of stenosis between the right ventricle and the pulmonary artery.
On exploration using a pump-oxygenator, a 2 ¡¿ 3 §¯ irregular sac was found which was arising from the right aortic sinus, blocking partially the outflow tract of the right ventricle. The perforations as well as the two small ventricular septal defects immediately below the aneurysm were closed by direct sutures. The postoperative recovery was uneventful.
The clinical as well as the catheterization data indicating a pulmonic stenosis in this patient is believed to be due to the presence of the aneurysmal sac in the outflow tract of the right ventricle which has not been reported in the literature. This patient also lacked the usual symptoms of a ruptured aneurysm i. e., sudden onset of symptoms, progressive course and continuous murmurs.
